Community Health Worker - Children and Youth Services


Job Type: Full-time

Description

We are expanding our team to serve the needs of our community!  Central Ozarks Medical Center (COMC) is excited to announce an opening for a Community Health Worker - Children and Youth Services.  COMC is a non-profit, federally qualified health center that has served Pulaski, Laclede, Camden and Miller counties for over 40 years with clinics located in Richland, Camdenton, and Osage Beach Missouri. COMC is looking for a team member who is a comfortable working with patients from a diverse background.  Must be a motivated, self-starter able to quickly adapt to new and changing conditions while maintaining a professional demeanor.  

The Children and Youth Services Community Health Worker (CHW) will assist and support children and youth in foster care, adopted from foster care, Division of Youth Services, involved in the juvenile justice system, and their families. The CHW will conduct in-reach, outreach and education activities to existing health center patients and non-health center patients to access primary medical, dental, and behavioral health care as well as conducting preventive assessments and screenings. The CHW will coordinate transitions, referrals and follow-up and thorough documentation and track all supports. This position required thorough documentation to support reporting requirements.  This position will float between our clinics as required.  

Essential Duties and Responsibilities include the following. Other duties may be assigned.


Community Health Worker – Children and Youth Services

  • ­­­­­­­­­­­­­­­­­­­­­­Assist Children and Youth in care as well as their families in accessing primary medical, dental and behavioral health care for children, with a focus on getting Children and Youth seen for required initial and follow-up care. 
  • Conduct PRAPARE Assessments to determine SDOH needs of children and foster families annually or more frequently, if change to foster child placement. 
  • Work collaboratively as part of the COMC Care Team as well as more broadly as a part of a inter-disciplinary care team for multiple agencies.
  • Conduct Suicide Risk Assessment via the Columbia Suicide Severity (C-SSRS) Assessment annually or more often as needed.
  • Provide care coordination, which may include but is not limited to facilitating care transitions, supporting the completion of referrals, and providing or confirming appropriate follow-up. 
  • Make referrals and connections to community resources to help individuals and families meet basic social needs.   
  • Build upon existing relationships with other community agencies and form new partnerships to best assist families. 
  • Assist with the development of marketing and promotional materials for outreach, application assistance and education activities.
  • Tracking and documentation of encounters, assessments, and referrals to measure outcomes and assist in reporting efforts.
  • Provide culturally and linguistically appropriate services and provide limited English proficiency applicants with information on how to obtain services in a preferred language. Ensure physical and other accessibility for people with disabilities.
  • Provide health insurance application and renewal assistance and facilitate enrollment in a health insurance program by providing fair, impartial, and accurate information. Maintain knowledge and expertise in eligibility, enrollment, and program specifications of the Medicaid and have some basic knowledge of the Qualified Health Plans (QHP).
  • Conduct monthly in-reach, outreach and education activities to existing health center patients and community residents to promote awareness about coverage options under Medicaid and the Marketplace.


Requirements

 Qualifications Requirements: 

  • Strong oral and written communication skills. 
  • Ability to work effectively and professionally in a fast-paced environment. 
  • Knowledge of and ability to work and engage with the uninsured, under-served and under-represented populations. 
  • Must have intermediate computer skills and be able to type at least 40 wpm. 
  • Ability and willingness to successfully complete formal CHW training through a COMC paid and approved program. 
  • Ability and willingness to successfully complete training and education on the Missouri Foster Care Program, Trauma Informed Training, Common Behavioral Health Conditions of children and youth, suicide prevention, and SUD.


Education and/or experience: 

High School diploma or equivalent required. Preferred: Lived experience with the Missouri Foster Care Program as a child, parent, adopted child, or through work or volunteer experience. Work or volunteer efforts with child advocacy programs, juvenile justice, or behavioral health organizations are also desirable. 

Language Skills: Ability to read, analyze, and interpret financial reports. Ability to respond to common inquiries or complaints from customers and regulatory agencies. Ability to effectively present information to top management.

Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. 

Other Skills and Abilities: Outgoing personality with strong phone etiquette skills, outstanding customer service skills. 

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This includes moderate physical effort consisting of occasionally lifting or moving over 35 pounds of weight. While performing the duties of this job, the employee is frequently required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 lbs. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Travel Requirement: To fulfill the responsibility of the position travel to all COMC locations may be required. Employee must have valid Missouri driver’s license and availability of a motor vehicle.

Description

 We are expanding our team to serve the needs of our community!  Central Ozarks Medical Center (COMC) is excited to announce an opening for a Psychiatrist.  COMC is a non-profit, federally qualified health center that has served Pulaski, Laclede, Camden, and Miller counties for over 40 years with clinics located in Richland, Camdenton, and Osage Beach Missouri.


RESPONSIBILITIES: This position is part of the behavioral health clinical team.  This position provides services to patients. The position participates in clinic performance improvements.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Evaluates patients by interviewing patients, family, and other persons; conducting physical examinations; observing behaviors; reviewing medical history and related documents;      selecting, administering, and interpreting psychological tests; ordering laboratory tests and evaluating results.

Develops treatment plans by determining the nature and extent of cognitive, emotional,      developmental, social, and behavioral disorders; establishing treatment goals and methodologies. 

Collaborates with Behavioral Health Director, Medical      Director, LCSWs, and other professional staff to assist the Behavioral      Health Director in developing a comprehensive program of therapy, evaluation, and treatment.

Supervises and collaborates with Psychiatric Nurse      Practitioners as assigned.

Treats patients by utilizing psychotherapeutic methods and medications; discussing progress toward goals with patients; providing instructions; monitoring the effect of medications; supervising staff provision of services.

Maintains interdisciplinary treatment by reviewing treatment plans and progress;      consulting and collaborating with primary care physicians, mental health therapists, nurses, and other health care providers.

Assures quality and safe service for patients and staff by enforcing policies, procedures,      standards, rules, Joint Commission for Accreditation of Health Care Organizations requirements, and legal regulations; participating in utilization reviews; remaining available for emergency consultations.

Improves staff competence by providing training and discussions regarding the interrelationship between psychosocial and physiological problems, psychotropic medications, crisis intervention, and related issues.

Maintains historical records by documenting symptoms, medications, and treatment events; writing summaries.

Provides clinical consultation to other providers within COMC as needed.

Responsible for accurate, timely, and clear medical record documentation.

Provides clear written and oral communication to patients/guardians, and employees.


Requirements

 

Qualifications Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.


Education and/or Experience: 


Full licensure to practice in the State of Missouri. 

Board certification from American College of Psychiatry and Neurology.

Requires an M.D. or D.O. 

Must have and maintain a current Controlled Substances Registration Certificate from the State of      Missouri, and the U.S. Drug Enforcement Administration.

Completion of 2-5 years experience in a clinic setting, preferred. 

Able to communicate with others to effectively convey information.

Social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base.

Language Skills: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the community. 


Mathematical Skills: Ability to work with all accounting concepts and budget(s). 


Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. 


Other Skills and Abilities: Must be able to perform each essential duty satisfactorily. 

Physical Demands: While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee is occasionally required to stand and reach with hands and arms. Specific vision abilities required by this job include close vision and the ability to adjust focus.

Travel Requirements: To fulfill the responsibility of the position travel to various areas of the region is required. The employee must have a valid Missouri driver’s license and the availability of a motor vehicle. Overnight travel may also be required in specific incidences of training, conferences, etc.

Work Environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Knowledge of, and compliance with, HIPAA and 340B regulations is required. Confidentiality of COMC proprietary information is mandatory. 


The noise level in the work environment is usually moderate.

Hours are flexible as approved by the Behavioral Health Director.

Is This Is Your First Time Being Pregnant? Here Are Some Helpful Pointers!


STOP CONSUMING THINGS THAT ARE BAD FOR YOU AND YOUR BABY.

These items include but are not limited to:

  • Alcohol
  • Caffeinated beverages, such as coffee or soda
  • Fish with high levels of mercury, like swordfish or shark
  • Undercooked meat
  • Raw eggs
  • Lunch meat
  • Hot dogs

...it's best to get a list from your doctor of everything you shouldn't eat, but these are a few good examples that will likely be on it.


REALIZE YOU'RE SUPPOSED TO GAIN WEIGHT AT THIS TIME.

According to the National Institute of Health:

"If you are underweight (BMI* less than 18.5), you should gain about 28 to 40 pounds,

normal weight (BMI of 18.5 to 24.9), you should gain about 25 to 35 pounds, overweight (BMI of 25 to 29.9), you should gain about 15 to 25 pounds, and obese (BMI of 30+), you should gain about 11 to 20 pounds."


CONSIDER TAKING PRENATAL VITAMINS, BUT ASK YOUR DOCTOR FIRST.

Many healthcare providers may advise you to take a prenatal vitamin every day, but it's best to ask your doctor what you should specifically do. Every mother-to-be is different.

Back-to-School Time is Approaching...Need Some Health Tips For Your Kiddos?


HOW HEAVY IS YOUR KIDDO'S BACKPACK?

When your kids are back in school they may be carrying heavy textbooks in their backpacks every day. That can be very hard on their back if the weight isn't distributed right. Make sure that they wear their backpack with both straps over their shoulders.


HAVE YOU TAUGHT YOUR KIDDO HOW TO AVOID LICE?

Teach your kids about the dangers of head lice and how they can easily spread. Tell them to avoid:

  • Sharing helmets during sports activities
  • Borrowing someone else's hairbrush at school
  • Borrowing a friend's hat


DO YOU PRACTICE GOOD HYGIENE WITH YOUR KIDDOS?

Your kid is going back to school and will be around other children. You probably don't want them to feel embarrassed because they didn't practice good hygiene, so make sure they know how to do so. Here are a few tips:

  • Teach them to wash their hair at least once every two or three.
  • Teach them to bathe regularly.
  • Teach them to brush their teeth at least twice a day, once in the morning and once at night.
  • Depending on the age of your child, if they have begun puberty, you may have them start wearing deodorant. 
  • Teach them to brush their hair every day.

HOW MUCH SLEEP IS YOUR KIDDO GETTING A NIGHT?

Your child is back in school and should be well-rested for all of the activities and knowledge they are about to soak up.


DOES YOUR CHILD WASH THEIR HANDS OFTEN?
